Concern Resolution Process

1. 1 concern per form only. If you have more than 1 concern, it will be returned back to you for review.

2. Your concern will go to the office and acknowledgement will be sent back to you within 24 hours. The concern will be reviewed by the office and will be presented at the following Board of Directors meeting if required. This process will be completed on a monthly basis.

3. Once a decision has been reached, communication to you with action, steps taken, and resolution (if required) will be provided.
